Ankle problems range from a rolled ankle last weekend to Achilles pain that has built up over months. I work out what is going on and build a plan back to walking, running and sport.

Common ankle problems I see

  • Rolled ankle sprains, including repeat sprains and an ankle that gives way.
  • High ankle sprains after a twisting injury in sport.
  • Achilles tendon pain with running, jumping or walking.
  • Pain at the front or back of the ankle with sport, landing or kicking.
  • Stiffness and weakness after an ankle fracture or time in a boot or cast.
  • Rehabilitation after ankle or Achilles surgery.

What the first appointment involves

The initial consultation is 45 minutes, one to one with me. It covers:

  • How the injury happened, and what brings on pain, swelling or giving way.
  • Ankle movement, stability, balance and calf strength.
  • How the ankle handles walking, hopping, running or sport, as appropriate.
  • Checks for signs that an X-ray is needed after an injury.
  • Screening for anything that needs a doctor’s review.

Most ankle sprains do not need an X-ray. Doctors and physiotherapists use a set of simple checks, called the Ottawa ankle rules, to decide when one is needed. If you need imaging or a medical opinion, I will explain why and organise the next step with you or your GP.

How rehabilitation works

  1. Understand. What is injured and what to expect.
  2. Protect. Support the ankle as needed in the early days, and keep it moving within comfort.
  3. Rebuild. Progressive balance, strength and hopping work for the ankle and calf.
  4. Return. Step back into running, changing direction and sport in stages.

When to get medical help first

Call 000 or go to the nearest emergency department now if you have:

  • an ankle or foot that looks deformed, or a foot that is cold, pale or numb after an injury
  • an injury that leaves you unable to take four steps
  • calf pain or swelling with chest pain or shortness of breath

See your GP promptly, or call healthdirect on 1800 022 222, if you have:

  • a sudden pop or kick-like pain at the back of the heel with trouble pushing off, which could be an Achilles rupture and needs assessment the same day
  • tenderness when pressing on the ankle bones or the outside of the midfoot after an injury, which can mean an X-ray is needed
  • calf pain, swelling or redness, especially after surgery, a long flight or time in a boot or cast
  • a hot, red, swollen joint with fever

Common questions

Protect it in the first few days, but long rest is rarely needed. Early, gradual movement and walking within comfort usually help more than keeping it still. What is right for your ankle is worked out at the assessment.

Repeat sprains are common, often because balance and strength were not fully rebuilt after the first one. Targeted balance, strength and hopping work is a large part of the plan.

Sometimes, especially when returning to sport after a sprain. It supports the rehabilitation rather than replacing it.
